Perceptions de femmes ayant vécu une agression sexuelle quant aux pratiques professionnelles favorables à leur bien-être psychologique pendant l’accouchement : une étude qualitative descriptive

Résumé

Introduction : Les femmes ayant vécu une agression sexuelle (AS) présentent un risque accru de détresse et de réactions traumatiques lors de l’accouchement. Certaines pratiques professionnelles en périnatalité peuvent contribuer à ces expériences.
 
Objectif : Explorer les perceptions de femmes ayant vécu une AS concernant les pratiques professionnelles pouvant contribuer à leur bien-être psychologique lors de l’accouchement.
 
Méthode : Une étude qualitative descriptive a été menée auprès de 11 participantes ayant rapporté une expérience d’AS antérieure à l’accouchement et ayant accouché au Québec (Canada). Le recrutement a été fait en ligne. Les entretiens ont été analysés selon la méthode d’analyse thématique par modèle.  
 
Résultats : Les résultats mettent en évidence 5 dimensions de pratiques professionnelles pouvant contribuer au bien-être psychologique des participantes : préparer l’accouchement avec la femme, soutenir son pouvoir d’agir, établir une relation de confiance sécurisante, adapter les soins pour réduire les risques de réactivation traumatique et préserver les liens significatifs autour de la naissance.
 
Discussion et conclusion : Les résultats suggèrent que, pour les femmes ayant vécu une AS, le bien-être psychologique durant l’accouchement est influencé par la qualité de la relation avec les professionnel·les, ainsi que par des pratiques favorisant le consentement, le sentiment de contrôle, la sécurité émotionnelle et la reconnaissance du vécu traumatique. Ils soutiennent l’importance d’un accompagnement humain, personnalisé et fondé sur une approche sensible aux traumas, ainsi que la nécessité de renforcer la formation continue des professionnel·les en périnatalité afin de favoriser des soins sécurisants et de réduire les risques de réactivation traumatique.
